Well, we know from the Clark Kent/Superman thing that most people aren't that observant,
so I guess the pregnancy would be a confusing factor. But then you've got Perry saying Lori looks just like Lois did, ten years ago...
oh, well, I'll stop trying to picture it in my head and just enjoy the story...
