I've never done it before, so have no suggestions. Whatever format is decided upon is fine with me.

BTW, in the past beta readers were recommended, since you don't want to give somebody a lousy gift. However, they aren't required. Better to have something up on time, than to not have something up at all. Also, in the past it was a HUGE SECRET who you were writing for.
